The Facility Land Ban Notification Form

The land ban form is required to accompany any hazardous waste manifest used for shipping hazardous waste(s) that do not meet the treatment standards specified in 40 CFR Subpart D or the prohibitions specified in 40 CFR 268.32 or RCRA Section 3001(d).

EMI is currently licensed to accept, or has pending licensure to accept the following waste codes subject to the above land ban regulations:

D004 (Arsenic)
D005 (Barium)
D006 (Cadmium)
D007 (Chromium)
D008 (lead)
D010 (Selenium)
