The
Virgin Material Origin Statement (VMOS) only replaces
a completed profile form and analysis for liquids and/or
solids contaminated with virgin fuel oils. Oils that are
waste products as a result of process use or mixing with
other product must be supported by analytical test
results and/or a completed waste
profile form that is approved by ENPRO Services of
Maine, Inc., prior to acceptance.

The Virgin Material
Origin Form is
divided into several sections and requires the following
information:

Customer (company
bringing the waste to EMI)

Purchase Order Number

Generator
Name

Generator
Mailing Address
-
Generator Contact
- Title
- Phone
- Site Location
Address
This is the address that the waste was actually
picked up from, not the mailing or main office
location.
- Generator USEPA ID
Number
This number should be issued for and represent the
physical location of a waste pick up. If your company
has a USEPA ID number for their main location, but the
waste is generated from a spill off-site, a temporary,
or possibly permanent USEPA ID number must be acquired
from the state environmental agency at which the waste
pick up occurred. EMI can assist you in obtaining this
number.
- Transporter
This field is for the name of the properly
licensed transporter that will be delivering your
waste to EMI.

Origin
The source of your virgin petroleum waste and type
of petroleum product

Process
Generating the Waste

Waste
Composition

(describe type of debris, such as rags absorbents etc.)

Certification
This section must be signed by the waste generator
or his/her authorized representative. No waste
stream(s) will be accepted at EMI without a proper
generator certification signature.
